On July 16, 2008, at approximately 1752 hours, I responded to 3843 Sutton Place Boulevard #708 (Sutton Place Apartments) where I met with witness, Anthony Lazzaro. I conducted an audio recorded interview with Mr. Lazzaro, The following is a synopsis of the interview: Mr. Lazzaro and suspect, Casey Anthony, are boyfriend and girlfriend. Mr. Lazzaro met Ms. Anthony on the Facebook website on or around May 21,2008. Since they became boyfriend and girlfriend, Ms. Anthony had spent several days and nights with Mr. Lazzaro at 3843 Sutton Place Boulevard #708. Ms. Anthony went to work with Mr. Lazzaro at 875 Woodbury Road (The Fusion Restaurant) on several occasions. Mr. Lazzaro works as a disc jockey at the Fusion Restaurant. Ms. Anthony assisted Mr. Lazzaro by managing the "Shot girls." "Shot girls" is common street terminology for women who sell individual shots of alcohol to patrons of the restaurant. From the beginning of June 2008 to July 15, 2008, Ms. Anthony lived with Mr. Lazzaro at 3843 Sutton Place Bouievard #708. Ms. Anthony brought victim, Caylee Anthony, to the residence during the day, but Caylee Anthony never spent the night at 3843 Sutton Place Boulevard. Mr. Lazzaro attends classes at Full Sail University during the day. Ms. Anthony told Mr. Lazzaro she worked at Universal Studios during the day. Mr. Lazzaro usually returned from his classes at approximately 1600 hours, and Ms. Anthony would be inside 3843 Sutton Place Boulevard #708. Ms. Anthony would cook and clean the residence. Mr. Lazzaro advised since June 2, 2008, until July 16, 2008, Ms. Anthony basically lived with him at 3843 Sutton Place Boulevard #708. Whenever Caylee Anthony was brought up in conversation, Ms. Anthony would make excuses such as "Caylee was at Disney World with "Zanny the nanny," Caylee was at Cocoa Beach with the nanny, and even my mother, Caylee is with my mother." Mr. Lazzaro never met "Zanny the nanny." Ms. Anthony brought Caylee Anthony to 3843 Sutton Place Boulevard on several occasions, Mr. Lazzaro described Ms. Anthony's interaction with Caylee Anthony as normal. Ms. Anthony fed, bathed, and showed affection to Caylee Anthony. Mr. Lazzaro recalled last seeing Caylee Anthony on June 2, 2008. Mr. Lazzaro, Ms. Anthony, and Caylee Anthony went to the swimming pool of the Sutton Place Apartments, Mr. Lazzaro took a photograph of Caylee Anthony with his cellular phone. Mr, Lazzaro showed me the photograph. Mr. Anthony swore the information he provided was true. A copy of the recorded interview was submitted into evidence. On July 17, 2008, at approximately 1852 hours, I responded to 8406 Fort Jefferson Boulevard to conduct an interview with witness, Jeffrey Hopkins II. I met with witness, Jeffrey Hopkins, and witness, Melissa Hopkins. I conducted a surreptitiously audio recorded interview with Jeffrey Hopkins and Mrs. Hopkins. The following is a synopsis of the interview: Jeffrey Hopkins II does not reside at 8406 Fort Jefferson Boulevard, and his cellular telephone is out of service. Mrs. Hopkins and Jeffrey Hopkins denied any knowledge of Casey Anthony and advised they knew all of Jeffrey Hopkins II's friends. Mrs. Hopkins and Mr. Hopkins advised on Juiy 16, 2008, at approximately 0500 hours, they received a telephone call on their home telephone (407-273-2789) from someone their caller identification system identified as "Anthony Casey 407-619-9286." The phone rang twice, and neither Mrs. Hopkins nor Jeffrey Hopkins answered the telephone. The caller did not leave a voicemail message. A copy of the recorded interview was submitted into evidence. On July 16, 2008, at approximately 2008 hours, I responded to 100 South Hughey Avenue (Orlando Police Department Headquarters) where I met with witness, Amy Huizenga. Ms. Huizenga was in the process of making a fraud report alleging Ms. Anthony had stolen and cashed several checks from her. Orlando Police Officer R. Smith completed a report under 08-307930. I conducted an audio recorded interview with Ms, Huizenga, The following is a synopsis of tile interview: Ms. Huizenga met Ms. Anthony at a party in the summer of 2007. Ms. Huizenga went away for a few months and was reacquainted with Mrs Anthony again in February 2008. Ms. Anthony began dating Ms. Huizenga's friend Ricardo Morales. Between February 2008 and late May 2008, Ms. Huizenga and Ms. Anthony spoke on the telephone and text messaged each other every day. Ms. Huizenga spent a large amount of time with Ms. Anthony and Caylee Anthony, Ms. Huizenga described Ms, Anthony's and Caylee Anthony's relationship as normal. Ms. Huizenga considered Ms. Anthony her best friend. Ms. Anthony told Ms. Huizenga she was going to take over the payments of her mother's home and arranged for Ms. Huizenga to move in with her. Months later, Ms. Anthony Incident Report ORANGE COUNTY SHERIFF'S OFFICE told Ms. Huizenga her father had been caught cheating on her mother and the agreement to take over her mother's home had been rescinded. Ms. Anthony told Ms. Huizenga she had ten or fifteen thousand dollars in the bank and planned on purchasing a home. Ms. Anthony invited Ms. Huizenga to move in with her. Ms. Huizenga moved into 232 North Glenwood Avenue with Ricardo Morales in April or May of 2008. Ms. Anthony brought Caylee Anthony to 232 North Glenwood Avenue on several occasions. Ms. Anthony even brought Caylee Anthony to a few adult parties held at night. In April 2008 or May of 2008, Ms. Anthony broke up with Ricardo Morales and began dating Mr. Lazzaro. When Ms. Anthony began dating Mr. Lazzaro, contact between Ms. Anthony and Ms. Huizenga fell to a text message or a telephone call once or twice a week. Ms. Anthony began spending most of her free time with Mr. Lazzaro. From the end of June 2008 to July 16, 2008, Ms. Huizenga did not see Caylee Anthony. Whenever conversation turned to Caylee Anthony, Ms. Anthony replied Caylee Anthony was at Sea Worid with the nanny or at Cocoa Beach with the nanny and Ms. Anthony's mother. Ms. Huizenga had never met the nanny. Ms. Huizenga, Ricardo Morales, and other friends took a trip to Puerto Rico during the week of July 12, 2008. Ms. Anthony borrowed Ms. Huizenga's car while Ms. Huizenga was in Puerto Rico. Ms. Anthony picked Ms. Huizenga and the others up from the airport during the evening of July 16, 2008. Later during the evening of July 16,2008, Ms. Huizenga received a telephone call from Ms. Anthony's mother. Ms. Anthony's mother told Ms. Huizenga the car Ms. Anthony uses had been found, but Ms. Anthony and Caylee Anthony could not be found. Ms. Huizenga agreed to take Ms. Anthony's mother to Mr. Lazzaro's residence. Ms. Anthony's mother picked Ms. Huizenga up and Ms. Huizenga led her to 3843 Sutton Place Boulevard #708. Once at 3843 Sutton Place Boulevard, Ms. Anthony's mother confronted Ms. Anthony. Ms. Anthony's mother asked where Caylee Anthony was, and Ms. Anthony told her Caylee was safe. Ms. Anthony's mother demanded Ms. Anthony take her to Caylee immediately. Ms. Anthony refused. Ms. Anthony's mother agreed to drive Ms. Huizenga home, and during the ride back, Ms. Anthony's mother questioned Ms. Anthony. Ms. Anthony did not provide any information on Caylee Anthony's whereabouts. Ms. Huizenga swore the information she provided was true. A copy of the interview was submitted into evidence. On July 17, 2008, at approximately 2128 hours, I responded to 232 North Glenwood Avenue where I met with witness, Ricardo Morales. I conducted an audio recorded interview with Mr. Morales. The following is a synopsis of the interview: During the Summer of 2007, Mr. Morales traveled from Tampa, Florida to attend a party in Orlando, Florida. Mr. Morales met with Ms. Anthony at the party. Mr. Morales met Ms. Anthony again in February 2008, and they began dating. Mr. Morales and Ms. Anthony dated until late April 2008, or early May 2008. While Mr. Morales and Ms. Anthony dated, they spent most of their time at 232 North Glenwood Avenue. Mr. Morales and Ms. Anthony went out to eat together one or twice, but most of the time they hung out inside of 232 North Glenwood Avenue. A majority of the time Ms. Anthony spent at 232 North Glenwood Avenue, Caylee Anthony was with her. Mr. Morales described the relationship between Ms. Anthony and Caylee Anthony as normal. Ms. Anthony and Caylee Anthony spent the night at 232 North Glenwood Avenue on several occasions. When they spent the night, Ms. Anthony, Caylee Anthony, and Mr. Morales shared a bed. After they ended their relationship, Mr. Morales wouid only see Ms. Anthony out at parties, or when she came to visit Ms. Huizenga who had moved into 232 North Glenwood Avenue. Mr. Morales last saw Caylee Anthony when she spent the night at his residence. Ms. Anthony and Caylee Anthony spent the night at 232 North Glenwood Avenue during the evening of June 9, 2008. Mr. Morales saw Caylee Anthony and Ms. Anthony during the morning of June 10, 2008, when he left for work. Mr. Morales swore the information he provided was true. A copy of the recorded interview was submitted into evidence. On July 17, 2008, at approximately 0811 hours, I responded to 789 Penlon Court where I met with Jeffrey Hopkins II. I conducted a surreptitiously audio recorded interview with Jeffrey Hopkins II. The following is a synopsis of the interview: Jeffrey Hopkins II went to high school with Ms. Anthony. Jeffrey Hopkins II had not seen Ms. Anthony for years until two or three weeks prior to July 16, 2008. Jeffrey Hopkins II ran into Ms. Anthony while they were eating with separate parties at 641 North Alafaya Trail (Orlando Ale House). Ms. Anthony and Jeffrey Hopkins II had a brief conversation and returned to their meals. Mr. Anthony had never met Caylee Anthony, and had no idea why Ms. Anthony called his parent's residence. A copy of the recorded interview was submitted into evidence. Copies of the recorded interviews were turned over to Corporal Melich. Incident Report ORANGE COUNTY SHERIFF'S OFFICE On July 17, 2008, I presented a court order and accompanying affidavit for Ms. Anthony's cellular telephone activity to the Honorable Judge Robert Evans. Judge Evans signed and sealed the court order and the accompanying affidavit. I immediately served AT&T Wireless with the court order by facsimile. I followed up service of the court order with a verification call to the AT&T Wireless Subpoena Compliance Center where I received verification AT&T Wireless received the court order. On July 18, 2008, I received a portion of Ms. Anthony's cellular telephone activity yia a secure email from AT&T Wireless. On July 19, 2008, I received another portion of Ms. Anthony's cellular telephone activity via a secure emall. The contents of the secure emails were turned over to Corporal Melich, and a copy was submitted into evidence. On July 21, 2008, I presented a court order and accompanying affidavit for Mr. Lazzaro's cellular telephone activity to the Honorable Judge Stan Strickland. Judge Strickland signed and sealed the court order and accompanying affidavit. I immediately served AT&T Wireless with the court order by facsimile. I followed up service of the court order with a verification call to the AT&T Wireless Subpoena Compliance Center, where I received verification AT&T Wireless received the court order. On July 22. 2008, I received Mr. Lazzaro's cellular telephone activity via a secure email from AT&T Wireless. The contents of the secure em all was turned over to Corporal Melich, and a copy was submitted into evidence. On July 24, 2008, at approximately, 0905 hours, I responded to 7777 Narcoossee Road (Johnson's Wrecker East). I was provided with a packet containing tow records for a white Pontiac (1998, 2 door, white, Pontiac, FL G63XV, ViN: 1G2JB1243W759664B) which was towed on June 30, 2008, and released to the owner on July 15, 2008. The packet contained the following items: a removal form, a private property tow form, an invoice, an Orange County Tag Talk System print out, Orange County Sheriffs Office Vehicle Impound form, a copy of an automobile title in the names of George Anthony and Cynthia Anthony, and a copy of George Anthony's driver license. I met with and conducted an audio recorded interview with witness, Simon Birch. The following is a synopsis of the interview: Mr. Birch is the facility manager of wrecker yard. Mr. Birch recalled seeing the white Pontiac inside of 7777 Narcoossee Road for several weeks. There is a high turnover of cars at the lot, so Mr. Birch became curious when the white Pontiac was still there after a few weeks. Mr. Birch walked over to the white Pontiac to look the car over and immediately smelled a foul odor. Mr. Birch has experienced bad smells coming from cars towed containing perishable foods, so he did not pay any further attention to the white Pontiac. During the morning of July 15, 2008, while Mr. Birch was working at 7777 Narcoossee Road, Mr. Birch was summoned by witness, Nicole Lett. Mrs. Lett was working in the front office of the tow yard and was dealing with two upset customers. Mr. Birch met with George Anthony and Cynthia Anthony. Ms. Anthony was upset with the cost of the tow, the length of time the car was stored, and requested a discount. Mr. Birch explained the cost of the tow and storage fees, and explained the notification process, to Cynthia Anthony and George Anthony. Mr. Birch refused to give a discount. George Anthony agreed to pay for the release of the white Pontiac, provided the white Pontiac's title, and his driver license. Mr. Birch took Mr. Anthony into the tow yard to get the car. As they walked, George Anthony mentioned to Mr. Birch his daughter had been missing for a while, now she will not allow him to see his granddaughter, and has told him several lies. When they reached the white Pontiac, . George Anthony used a key to unlock the white Pontiac's driver side door. When the door opened, a very foul odor came from inside of the white Pontiac. Mr. Birch immediately recognized the odor as the same odor he smelled in a car stored on the lot in which a man committed suicide inside. The man was reported to have remained deceased inside of that car for approximately five days. The smell from the white Pontiac was not as strong as the other car. Mr. Birch stated "That's rotten." George Anthony did not reply. George Anthony and Mr. Birch went to the white Pontiac's trunk in an attempt to locate the source of the odor. George Anthony opened the trunk and he and Mr. Birch observed a white garbage bag inside of the trunk. Flies also came out of the trunk. Mr. Birch opened the garbage bag and observed several papers, a pizza box, and maggots. Mr. Birch threw the garbage bag over a fence near the tow lot's dumpster. George Anthony noticed the white Pontiac's fuel gauge read empty. George Anthony advised he had a gas can in his car, and walked out to the car he arrived at 7777 Narcoossee Road in. Moments later, George Anthony returned to the white Pontiac with a gasoline can, apologized for Cynthia Anthony being upset, poured the gasoline into the white Pontiac and drove away from the 7777 Narcoossee Road. Incident Report ORANGE COUNTY SHERIFF'S OFFICE Mr. Birch identified Gary Ridgeway as the driver who towed the white Pontiac to 7777 Narcoossee Road. Mr. Birch swore the information he provided was true. A copy of the recorded interview was submitted into evidence, and a copy of the recorded interview was turned over to Corporal Melich. I met with an conducted an audio recorded interview with witness, Gary Ridgeway. The following is a synopsis of the interview: Mr. Ridgeway is a tow truck driver for Johnson's Wrecker out of the 7777 Narcoossee Road tow yard. On June 30, 2008, at approximately 0850 hours, Mr. Ridgeway was on duty, and was dispatched to 7501 East Colonial Drive (Amscot Financial) to tow a car. On June 30, 2008, at approximately 0906 hours, Mr. Ridgeway arrived at 7501 East Colonial Drive. Mr. Ridgeway met with Catherine Sanchez who was an Amscot employee. Mrs. Sanchez pointed out the car and signed the private property tow form. Mr. Ridgeway recalled the white Pontiac was parked next to the dumpster. The white Pontiac was parked with the front of the car towards the curb. Mr. Ridgeway hooked up the white Pontiac and towed the car away at 0916 hours on June 30, 2008. Mr. Ridgeway returned to 7777 Narcoossee Road with the white Pontiac, unhooked the white Pontiac and went back into service at 1000 hours on June 30, 2008. Mr. Ridgeway did not notice any damage to the car, and did not smell any odors coming from the car. Mr. Ridgeway recalled having a cold on June 30, 2008, and could not smell anything. Mr. Ridgeway identified the private property tow form as the form he filled out and had Mrs. Catherine Sanchez sign. Mr. Ridgeway swore the information he provided was true. A copy of the recorded interview as submitted into evidence, and a copy of the recorded interview was turned over to Corporal Melich. On July 24, 2008, at approximately 1026 hours, I responded to 7051 East Colonial Drive (Amscot Financial) where I met with witness, Catherine Sanchez. I conducted an audio recorded interview with Mrs. Sanchez. The following is a synopsis ofthe interview: Mrs. Sanchez is the branch manager of 7051 East Colonial Drive (Amscot Financial). On June 27, 2008, at approximately 0700 hours, Mrs. Sanchez arrived at 7051 East Colonial Drive for her shift. Mrs. Sanchez observed a white Pontiac parked next to the business' dumpster, but took no action. On June 28, 2008, at approximately 0700 hours, Mrs. Sanchez arrived at 7051 East Colonial Drive (Amscot Financial) to begin her shift. Mrs. Sanchez observed the white car still parked next to the dumpster. Mrs. Sanchez notified the Amscot corporate office of the white Pontiac's presence. The Amscot corporate Office instructed Mrs. Sanchez to call law enforcement to ensure the white Pontiac was not stolen, and if the white Pontiac was not stolen wait one additionai day and have the white Pontiac towed away. Mrs. Sanchez called the Orange County Sheriff's Office and gave the white Pontiac's license pi ate to the operator. Mrs. Sanchez was advised the white Pontiac was not stolen. On June 30, 2008, at approximately 0700 hours, Mrs. Sanchez arrived at 7051 East Colonial Drive (Amscot Financial). Mrs. Sanchez observed the white Pontiac still parked next to the dumpster. Mrs. Sanchez called Johnson's Wrecker and requested the white Pontiac be towed away. A short time after Mrs. Sanchez's call, a tow truck driver arrived. Mrs. Sanchez pointed out the white Pontiac to the driver, singed a form, and the driver towed the white Pontiac away. I showed Mrs. Sanchez the Johnson's Wrecker private property tow form, and she verified the signature on the form belongs to her. Mrs Sanchez swore the information she provided was true. A copy of the recorded interview was submitted into evidence, and a copy was turned over to Corporal Melich. Mrs. Sanchez pointed out where tile white car was parked. The following is description of the area: 7501 East Colonial Drive is in a small business plaza which sits on the northeast corner of East Colonial Drive and Goldenrod Road. 7501 East Colonial Drive's lobby faces west towards Goldenrod Road. 7051 East Colonial Drive has entry doors on the west and south sides. 7051 East Colonial Drive's dumpster is fenced in and on the northwest portion of the western parking lot. There are two white striped parking spaces to the west of the dumpster. Mrs. Sanchez pointed out the parking space closest to the dumpster. This parking space is to immediate western side of the dumpster. I photographed the area. The photographs were submitted for analysis. On July 24, 2008, at approximately 1137 hours, I responded to 500 Wilmer Avenue (Johnson's Wrecker) where I met with witness, Nicole Lett. I conducted an audio recorded interview with Mrs. Lett. The following is a synopsis of the interview: During the morning of July 15, 2008, Mrs. Lett was working the front office of 7777 Narcoossee Road (Johnson's Wrecker East) when George Anthony arrived. George Anthony inquired about the cost and requirements to recover the car from the tow yard. Mrs. Lett told George Anthony what documents he would need, the cost. and Incident Report ORANGE COUNTY SHERIFF'S OFFICE advised him Johnson's Wrecker only accepted cash. George Anthony advised he did not have cash, would go get cash, and would return at a later time. Hours later on July 15, 2008, George Anthony returned to 7777 Narcoossee Road to recover the car. George Anthony was accompanied by Cynthia Anthony. Cynthia Anthony debated the notification time and the price for tile release of the white Pontiac. Cynthia Anthony appeared to have been upset. Mrs. Lett summoned Mr. Birch to speak with George Anthony and Cynthia Anthony. George Anthony provided Mrs. Lett with a title for the white Pontiac, his driver license, and the money for the release of the white Pontiac. Mrs. Lett completed the transaction and walked to the rear patio area to smoke a cigarette. As Mrs. Lett stood on the rear patio, Mr. Birch and George Anthony walked through the yard. As they walked through the yard, Mrs. Lett overheard George Anthony telling Mr. Birch he had just found his daughter, but his granddaughter was missing. I showed Mrs. Lett the packet of forms I received at 7777 Narcoossee Road. Mrs. Lett identified the forms she filled out or created, dated, and initialed them. The following is a list of forms Mrs. Lett filled out: the removal form, the invoice, and the Orange County Tag Talk System print out. Mrs. Lett created the copy of the automobile title in the names of George Anthony and Cynthia Anthony, and the copy of George Anthony's driver license. Mrs. Lett swore the information she provided was true. A copy of the recorded interview was submitted into evidence. and a copy was turned over to Corporal Melich. On July 24, 2008, I requested an investigative subpoena from the State Attorney's Office for financial records involving Casey Anthony from Amscot Financial Incorporated. On July 24,2008, I received and served the Investigative Subpoena to Amscot Financial Incorporated by facsimile. On July 24, 2008, I received the Amscot Financial records. A copy of the financial records was submitted Into evidence, and a copy was turned over to Corporal Melich. On July 24, 2008, I received Mr. Lazzaro's cellular telephone activity via a secure emall from AT&T Wireless. The contents of the secure emall was turned over to Corporal Melich, and a copy was submitted into evidence. |

Later during the evening of July 16, 2008, Ms. Huizenga received a telephone call from Ms. Anthony's mother. Ms. Anthony's mother told Ms. Huizenger the car Ms. Anthony uses had been found, but Ms. Anthony and Caylee Anthony could not be found. Ms. Huizenga agreed to take Ms. Anthony's mother to Mr. Lazzaro's residence." The correct date for both listings is "July 15, 2008." I discovered during the synopsis of the synopsis of the interview I conducted with witness, Jeffrey Hopkins II, I erroneously listed the following: "Ms. Anthony and Jeffrey Hopkins II had a brief conversation and returned to their meals.
