Transcribed by www.acandyrose.com (from .pdf file) Orange County Sheriff's Office Case # 08-CF-10925 Discovery Documents Set#2 (Pages 35-38) SUPPLEMENT #6 - INCIDENT REPORT (07/18/08): On July 17, 2008, I obtained sworn audio recorded statements from the following witnesses: Mr. Danny Colamarino, Ms. Maria Kissh, Mr. Jonathan Daly and Mr. Sean Daly. I met with Mr. Colamarino at his workplace, a Tattoo parlar located at 2818 S. Orange Ave. Orlando, Florida. Mr. Colamarino is the owner of Cast Iron Tattoos, and is an aquaintance of Defendant Mrs. Casey Anthony. Mrs. Anthony has been a customer of Cast Iron Tattoos for approximately a year and a half to two years, and regularly visits the establishment. The following is a synopsis of Mr. Colamarino's statement. Mrs. Colamarino last saw Mrs. Anthony on July 15, 2008, between 1400 hours and 1600 hours at Cast Iron Tattoos. Mrs. Anthony scheduled an appointment for Saturday July 19, 2008, for her and a friend to get a Tattoo. Mrs. Anthony had a Tattoo done on July 2nd or 3rd; the tattoo had the words "Bella Vita", on her shoulder blade. "Bella Vita" means "Beautiful Life." Mrs. Anthony arrived at the parlor in a Maroon or faded red four (4) door compact car, possibly a Toyota. Mrs. Anthony mentioned the car belongs to a friend; she borrowed it because she was going to pic up friends at the Orlando International Airport from a flight out of Puerto Rico, and she could not fit her friend luggage in her car. Mrs. Anthony was suppose to go on the Puerto Rico trip with her friends, but was unable due to financial reasons. Mr. Colamarino last saw Mrs. Anthony's daughter Caylee at his parlor, approximately three (3) to six (6) months ago. Mr. Colamarino asked about Caylee; Mrs. Anthony told him that she would bring Caylee for her appointment on July 19, 2008. I met with Ms. Kissh at her residence 1910 Lake Cecile Dr. Kissimmee, Florida. Mrs. Kissh is an aquaintance of Mrs. Anthony. The following is a synopsis of Ms. Kissh's statement. Mrs. Kissh met Mrs. Anthony through her boyfriend "Clint," who is a close friend of Mrs. Anthony's boyfriend Tony Lazzaro. Ms. Kissh last saw Mrs. Anthony with Caylee, on June 10th, 11th, or 12th, 2008, at Tony's apartment. Ms. Kissh last saw Mrs. Anthony on June 20th at Tony's place, where she spent many nights without Caylee, after June 9, 2008. Mrs. Anthony told Ms. Kissh that Caylee was with her "Nanny" who is a friend of hers. Mrs. Anthony never mentioned the "Nanny's" name. Mrs. Anthony mentioned that Caylee's father was deceased. Mrs. Anthony lived at her parent's house off Narcoossee Road, and that her parents moved out the house sometimes in early July. Mrs. Anthony worked at Universal Studios. Mrs. Kissh feels that Mrs. Anthony had a lack of attention or care towards Caylee. I met with Jonathon and Sean Daly at Cast Iron Tattoo. Jonathan and Sean have been friends with Mrs. Anthony for approximately 8 years. Jonathan and Sean mentioned that for the past months, Mrs. Anthony has distanced herself from her usually crowd of friends, and was "hanging out" with new friends. The following is a synopsis of Jonathan's statement: Jonathan last saw Mrs. Anthony early July, a couple of days before getting her Tattoo. Jonathan last saw Caylee several months ago, possibly at her birthday. Jonathan has heard the name of Ricardo, being Mrs. Anthony's current boyfriend. Mrs. Anthony was known to tell lies. Jonathan had heard Caylee's father was in the military, and Caylee was a conceived out of a one night stand. The following is a synopsis of Sean's sworn statement; Sean last saw Mrs. Anthony on July 5th at the Buffalo Wild Wings on Alafaya Trail. Mrs. Anthony was accompanied by her boyfriend Tony. Sean was told by a friend, Annie Downing, that Mrs. Anthony and Caylee spent the night of June 9, 2008, with a guy by the name of Ricardo. Sean mentioned Mrs. Anthony was known to tell lies. On July 16, 2008, at 0258 hours, Sean received a missed call from Mrs. Anthony's cellular phone number (407) 619-9286. I, Detective Rivera, witnessed the missed call from Mrs. Anthony phone number, displayed on the screen of Sean's phone. On July 17, 2008, I recieved a phone call from Mr. Mark Hawkins; he advised he knows Mrs. Anthony. Mr. Hawkins is an active duty Marine stationed in 29 Palms, California. Mrs. Anthony and Mrs. Hawkins were communicating on almost a daily basis via telephone and Internet. Approximately a week ago from July 17, 2008, Mrs. Anthony told Mr. Hawkins that she was planning a trip to california and she needed to tell him something. Mrs. Anthony said she told her mother and brother, and she did not know how he would react about what she had to tell him. Mr. Hawkins heard that Caylee's father was deceased, and he was a Security Guard at Universal Studios. Mr. Hawkins mother, Mrs. Ann Marie Chase knows Mrs. Anthony. They last saw each other at the Target store on Waterford Lakes. Mr. Hawkins provided Mrs. Chase's phone number (407) 467-8363. Mrs. Chase is due to return from a cruise on July 18, 2008. I spoke via telehone with Mrs. Chase who advised the following; Mrs. Chase last saw Mrs. Anthony at the Target store on July 10, 2008.
