Transcribed by www.acandyrose.com (from .pdf file) Orange County Sheriff's Office Case # 08-CF-10925 Discovery Documents Set#2 (Pages 39-42) SUPPLEMENT #7 - INCIDENT REPORT (07/18/08): On July 17, 2008, I was requested by child abuse Detective, Corporal Yuri Melich (2684) to meet with possible witnesses at the Cast Iron Tattoos Shop 2818 South Orange Avenue Orlando, Florida. On July 17, 2008, at approximately 1345 hours I arrived at 2818 South Orange Avenue and met with (Other#1) Mr. Bobby Lee Williams and conducted an interview. The following is synopsis of the interview: Mr. Williams stated he knows Ms. Casey Anthony from coming into the tattoo shop and getting tattoos. He is a tattoo artist and he completed a tattoo on her left shoulder that read "Bella Vida" on 2 July 2008 around noon time. This took approximately 30 minutes to complete. While she was there they talked about a new guy she had just started dating. The guy wasn't at the shop and she informed him the guy was in New York at a Yankees baseball game. Mr. Williams said the last time he saw Ms. Anthony was 15 July 2008. She came to the shop and engaged in conversation with his boss (Danny Colamarino). She was asked by his boss how her daughter was doing to which she replied she was fine and she was with the nanny. She then scheduled a tattoo appointment with him for Saturday July 19, 2008. She informed him that she was bringing a friend but she never told him who that friend would be. On July 17, 2008, at approximately 1403 hours while at 2818 South Orange Avenue I met with (Other#2) Mr. Lance Charles White and conducted an interview. The following is synopsis of the interview: Mr. White said he knows Casey Anthony because she use to come to the shop often. He is teaching Shawn Daly to do piercing and both Shawn and John Daly (co-worker) knows Casey from High School. He knows Casey has a daughter and the last time he saw her with the daughter was approximately 3 months ago in the shop. She has been in the shop several times since then but never with her daughter. Mr. White said Casey Anthony was last in the shop on Tuesday July 15, 2008. While at the shop she was engaged in conversation with Mr. Danny Colamarino. During the conversation she mentioned she was moving out of her parents house. She stated that her daughter was currentfy with a nanny. Danny asked her to bring her daughter to the shop so everyone could see her. She said she would bring her pass soon. She told them that her life was a little hectic right now but she did not seem to act any differently than she normally did when she was around. She made an appointment with Bobby Williams for Saturday July 19, 2008. Mr. White said Casey gave him a hug and this was the last time he saw her. On July 17, 2008, at approximately 1600 hours I arrived at 3440 North Goldenrod Road (Cranes Landing Apartment) Winter Park, Florida and met with (Other#3) Mr. Matthew Allen Crisp and conducted an interview. The following is synopsis of the interview: Mr. Crisp said he know Ms. Casey Anthony because the two of them attended Colonial High School together and have kept in contact since that time. He said the last time he saw Ms. Anthony was July 7, 2008 around noon time. He ran into her at Subway Sandwich Shop off of University/Goldenrod Road in Winter Park, Florida. They engaged in conversation about her boyfriend being out of town and her daughter. She stated her daughter was currently in Sanford, Florida with a babysitter. Her demeanor seem normal during the conversation and she informed him she was picking her daughter up later that afternoon. Mr. Crisp added on June 19, 2008, he received a call from Ms. Anthony and talked to her about leasing an apartment at his complex (Cranes Landing Apartments). She informed him the apartment would be for her boyfriend (Mr. Anthony Lazzaro) and he wanted to get out of his current lease with Sutton Place Apartments. The three of them met later that afternoon and completed a contact form. While at Cranes Landing Apartment complex on June 19, 2008 they did not have Ms. Anthony's daughter with them. Ms. Anthony demeanor was jovial, as if she was glad to see him. Mr. Lazzaro's demeanor was cordial but anxious to get out of his current lease. No lease agreement was signed on this date. Mr. Crisp said he called Ms. Anthony later in the week to inquire about the lease but she seem interested only in going to some club. She invited him to the club but he turned down the invitation. Mr. Crisp said yesterday morning July 16, 2008, he received a text message from Ms. Anthony's phone with the following message, "Casey has been missing for 32 days now, please jf you have any information call my number". Mr. Crisp said he text her back and asked if she was kidding. She replied back with a text, "No, its a long story and I'll tell you about it later". This was the last time he heard from Ms. Anthony. On July 17, 2008, at approximately 1855 hours I went to 541 West Highway 436 (Chucke-Cheese Restaurant) Orange County Sheriff's Office Case # 08-CF-10925 Discovery Documents Set#2 (Pages 39-42) SUPPLEMENT REPORT #7 (07/18/08) Altamonte Springs, Florida and I met with (Other#4) Ms. Dannielle E. Lucey-Austin (manager) to pick up a VHS video tape that recorded the front entrance to the restaurant for July 15, 2008. The time frame covered on the tape is 11:00 a. rn. and 1:00 p.m. Ms. Lucey-Austin signed over release of the tape to me on the Orange County Sheriff's Office Property Form. On July 18, 2008, I released the VHS video tape obtained from Ms. Lucey-Austin, the recorded audio statements obtained from Mr. Williams, Mr. White and Mr. Crisp over to Child Abuse Detective, Corporal Yuri Melich (2684).
